About Us
Reach PCC is an ACO created with the goal of engaging physicians and patients to improve healthcare outcomes and reduce unnecessary and/or wasteful healthcare spending for Medicare beneficiaries. Reach PCC is physician-led and focused primarily on independent primary care providers (PCPs) and specialists, and is currently operating in the following states: New Jersey, New York, Texas, Louisiana, Tennessee, Mississippi, Oklahoma, Kansas, and Kentucky, with plans for further expansion.
Our Vision
Reach PCC sees itself partnering with providers across the country – ones who are committed to their patients – in order to mitigate the rising healthcare costs. We believe that a well-functioning micro healthcare model takes into account all facets of patients’ lives holistically, and comes up with individualized strategies that tackle all the various challenges that patients face on a day-to-day basis. On the macro level, we believe that formidable and cost-efficient healthcare prevents before it treats, and the most feasible way to do this is through proactive care.
Through its partners, Reach PCC is engaged in population health management and partners with primary care providers offering wrap-around technology and services to help introduce greater patient engagement and better care. This is achieved by helping introduce programs and services that PCPs may not be currently providing to patients that need them. These services and programs include Annual Wellness Visits (AWVs), Advance Care Plans (ACP), Chronic Care Management (CCM), Behavioral Health Integration (BHI), and house calls for homebound patients.

How We Lower Healthcare Costs
Our approach is simple. We just provide our patients with the full suite of services that they are covered for, and make sure that we make medical professionals available to them 24/7. We also bolster up our patients’ primary care with crucial services targeting behavioral health and chronic care management, and implement these with the help of remote patient monitoring, telemedicine and house calls for the sickest patients. Our partner providers are those dedicated providers who stay one step ahead, and choose to prevent rather than merely treat.
